Healthcare is often fragmented and difficult to navigate, especially for those facing poverty, disability, discrimination, or geographic isolation.Every day, people living with chronic illness face overwhelming challenges after diagnosis: confusing healthcare systems, delayed referrals, missed appointments, financial hardship, lack of transportation, and no clear next step. For many patients, especially those in Indigenous, rural, and underserved communities, these barriers are even greater.
That’s where we come in. We believe every patient, caregiver, and loved one deserve to have support when they need it. Affordability should not be a barrier. Every dollar raised goes towards ensuring that care navigation and support is accessible to every patient and their loved ones.
The NPH Foundation is a 501(c)(3) nonprofit organization dedicated to improving health equity and patient outcomes through patient navigation.

Navigation is not medical treatment—it is support, coordination, and guidance that helps patients successfully move through complex healthcare systems and physical and emotional barriers.
Through patient navigation, the NPH Foundation helps:







...to timely, high-quality healthcare, including financial, geographic, cultural, and systemic obstacles
...to coordinated care and supportive services, especially for medically underserved populations
...to patients who are uninsured, underinsured, low-income, or otherwise vulnerable
...and education for Indigenous communities, minority populations, rural residents, older adults, and individuals with disabilities
...by offering resources, training, and support to navigators and care teams
...for all people, regardless of race, ethnicity, gender, age, disability status, sexual identity, or cultural background
