Who We Are
Healthcare is often fragmented and difficult to navigate, especially for those facing poverty, disability, discrimination, or geographic isolation.Every day, people living with chronic illness face overwhelming challenges after diagnosis: confusing healthcare systems, delayed referrals, missed appointments, financial hardship, lack of transportation, and no clear next step. For many patients, especially those in Indigenous, rural, and underserved communities, these barriers are even greater.
That’s where we come in. We believe every patient, caregiver, and loved one deserves to have support when they need it. Affordability should not be a barrier. Every dollar raised goes towards ensuring that care navigation and support is accessible to every patient and their loved ones.
The NPH Foundation is a global 501(c)(3) nonprofit organization dedicated to advancing health equity and improving patient outcomes through patient navigation. Our work facilitates access to patient navigation by funding services for those impacted by illness who would otherwise be unable to receive the guidance and support they need. Through education, we empower individuals, providers, healthcare organizations, policymakers, and governments with the knowledge needed to recognize patient navigation as an essential component of high-quality, patient-centered care. By advancing research, we are committed to demonstrating the impact of patient navigation while strengthening its long-term sustainability, relevance, and adoption across the global healthcare ecosystem.
What is Patient Navigation
Patient navigation is a proven, compassionate approach that helps individuals:

Navigation is not medical treatment—it is support, coordination, and guidance that helps patients successfully move through complex healthcare systems and physical and emotional barriers.
